Anna is Alex's first love, so he feels compelled to maintain it. In his eyes, if it fails it reflects on him as a person. He cannot accept that failure, so he goes back to Anna. Anna now knows this and she uses it to control him. Meanwhile he is missing out on the best sex he could ever have. He knows it and it's torturing him.
